This policy explains what Midtown East Physical Therapy collects through this website, why we collect it, and what we do with it. It covers this website only. Information you give us as a patient, in the office or during treatment, is handled under our clinical privacy practices, which are separate from this policy and which we provide to you directly.

Information You Give Us

Our contact form asks for your name, email address, phone number, how you heard about us, and a message. Our feedback form asks for your name, email address and your comments. You choose what to put in these fields, and you can call the office at (212) 922-0044 instead of using either form.

Contact form submissions are transmitted over an encrypted connection to a form-handling service operated by our web provider, Medi-Edge Marketing, and delivered to the practice by email. The feedback form works differently: it opens a message in your own email program with your answers filled in, and nothing is sent anywhere until you press send in that program.

Please Do Not Send Health Information

Email and web forms are not secure channels for medical information. Please use this form for general information purposes only. Do not send personal health information through this form. Specific patient care must be addressed during your appointment. If you need to discuss symptoms, diagnoses, treatment or anything else about your care, call us at (212) 922-0044 and we will take it from there.

Information Collected Automatically

Like most websites, our hosting provider records standard technical information when a page is requested, such as the IP address making the request, the browser and device type, the page requested and the time. This is used to keep the site running and secure. We also use Google Analytics to understand how visitors find and move through the site in aggregate. We do not use this information to identify you, and we do not sell it.

Third-Party Services on This Site

A small number of Google services run on these pages, and they may set cookies or receive your IP address when they load:

• Google reCAPTCHA protects the contact form from automated spam.
• Google Maps provides the maps to our two offices.
• Google Business Profile reviews are read live so the testimonials you see are the ones currently published on Google.
• Google Analytics measures site traffic, as described above.

Their handling of that data is governed by Google's own privacy policy. We do not run advertising trackers on this site, and our fonts are served from our own domain rather than from a third party.

How We Use and Share Information

We use what you send us to answer your question, schedule or manage your appointment, and improve the site. We do not sell your information or share it for anyone else's marketing. We share it only with the service providers who operate this website and our email on our behalf, and where the law requires it.

Retention, Security and Your Choices

We keep website enquiries only as long as we need them to respond and to keep a record of the request, and we take reasonable measures to protect them. No method of transmission over the internet is completely secure, so please keep the note above about health information in mind. You can ask us what website information we hold about you, ask us to correct it, or ask us to delete it, by writing to the address below. You can also refuse or delete cookies in your browser settings, though parts of the site such as the maps may not work as intended if you do.

Children

This website is intended for adults. We do not knowingly collect information through it from children under 13. If you believe a child has sent us information through this site, contact us and we will delete it.

Changes to This Policy

If we change how this website handles information, we will update this page and the date at the top. This version takes effect on August 3, 2026.
